Expanding the Tracking Cube across sectors in Queensland (QMHC Achieving Balance)

Expanding the Tracking Cube across sectors in Queensland (QMHC Achieving Balance)

Creating a home-point of coordination within community and healthcare systems.

Three regions of Queensland will use the Tracking Cube to become homepoints of coordination for collecting information and supporting at-risk children locally. Devolving coordination of care from specialists to trusted care professionals places community at the centre of support.

Sites will enhance their capacity to screen, monitor, and support at-risk children, reaching into specialist healthcare sites to access services for children with more complex comorbid neurodevelopmental needs.

We aim to:

  • Identify individual vulnerability by documenting risk and protective factors along with the social determinants of health in Tier 2 and 3.
  • Promote community engagement in harm reduction by asking about prenatal alcohol exposure and engaging community to support the reduction of PAE in culturally responsive ways.
  • Enable collective action at a systemic level by leveraging local expertise. By making community organisations the home-point of coordination we harness the power of trusted care professionals who know the children most.

The result will be improved coordination and linkage across systems, improved identification of FASD, improved workforce efficiency, capacity, and capability, and strengthened locally driven community-led support. Ultimately, children will be diagnosed faster and supported sooner.
